Re year over year percent changes, think about currency issues too. We track the price in original currency, not just the finally paid price in our currency, because the currency fluctuates so much that it can easily make a sub's price increase seem a few percent higher or lower than the publisher is actually asking for and could totally throw off our prediction for the next fiscal year. And then when we get those "rate adjustments" (we use EBSCO for many serials), we have to track the original currency of those too in order to predict the price in the next year.

As a Canadian library, this issue affects the vast majority of our subs, but even American libraries probably have some subs in other currencies that this will be an issue for.

Here is my template, I look over it monthly to make sure everything is on track.  Adding current use data and percent changes year by year helps when you get those questions about a database out of the blue.
