[Eril-l] Mechanism for soliciting journal title feedback?

Hi all,

I've been searching my archives and shared web pages and I'm having a hard
time finding what I'm looking for.

I know several libraries used a type of form to solicit feedback from
faculty/community about journal retention decisions. If someone did this,
could you send me a link to your page if still active or share with us the
software/program you used to create the feedback mechanism?

I'm playing around with Google Forms but not getting the look I want.
