The one I used is this:
Title Inside Higher Education
URL https://www.insidehighered.com/
Host www.insidehighered.com<http://www.insidehighered.com>
HJ www.insidehighered.com<http://www.insidehighered.com>
Domain insidehighered.com
DJ insidehighered.com
There are two basic problems with it: the first is that the login uses a different domain, tinypass.com (specifically id.tinypass.com), which means that part doesn’t work. You don’t need to login to get access (we got the IP range version), but I am confident that some users will still try and fail and complain.
The second is that a number of the images do not load, and as such there’s some weird spacing (the ads also don’t load, but that doesn’t bother me so much).
It’s also possible that my stanza is overcomplicated, but I don’t have direct access to our EZProxy, so I can’t tinker with it easily. Our consortium does it for us, and it’s a bit challenging have to send a request for a change, test it, make another request, etc., especially since I’m not entirely sure what’s necessary and what isn’t.

I'm working on setting up access to our new subscription now. This is the stanza the vendor provided. What is the stanza you created? This is not what I would have created as a stanza myself back when we were self hosted but since it's what the vendor provided I passed it along to OCLC.
Title Inside Higher Ed
URL https://www.insidehighered.com
Domain insidehighered.com<http://insidehighered.com>
We have hosted EZProxy and OCLC added this as is to our file. It seems to be working from the EZproxy side of things in that I hit our log in and then get passed through to Inside Higher Ed. But something isn't correct and our access set up isn't fully working yet and I'm not sure if it's the stanza and the EZproxy side of things or if it's on the Inside Higher Ed side of things. I'm waiting to hear back from the support person at Inside Higher Ed who is assisting with our set up.

Has anyone else gotten a subscription to the newly paywalled Inside Higher Ed? We’re looking to get it set up for EZProxy, but the vendor doesn’t have a stanza (and I’m not sure if they’re going to make one). I’ve made one following the “roll your own” directions that OCLC gives out, but it’s still pretty creaky, and I was wondering if anyone had had better luck with it.
