We also have something very similar, "Report a Broken Link", using a customlink.
We used a Google form, following instructions widely available on the web for configuring fields to be prepopulated.
Here are our key customlink settings:
Category: Document Delivery

Those entry.#s are the trick that various websites can tell you how to find - they are specific to your Google form.
FYI, if anyone interested in this thread is an EDS customer, there is a much better list to discuss this on, the EDS partners listserv. There are many very experienced customers there who have done all these kinds of things and can help answer questions. Ask EBSCO support how to get on that list as it is restricted to customers.

Related to this, we will be going live this summer too, and we are wondering whether any institutions have found a solution to the following problem we've identified with the "assess options" dropdown menu. Specifically, we would like to have our "report an issue" and "find full text" links appear below the PDF and Online full text links (see screenshot below). Is anyone aware of a workaround to re-order these links?

We’re planning to migrate to the new EBSCOhost UI over the summer and I’m trying to see what I would need to communicate out to our users before the semester ends and folks leave for the summer.
Links will redirect and personal accounts will use the same login. I know that users with search alerts set up will have to recreate their search alerts on the new UI. The migration of saved searches and custom folders should be coming out this month.
For those who have already migrated to the new EBSCOhost UI, did you encounter any issues with the migration and the new UI?
